Consent and Fee Agreement
Consent
1. I understand that my coach does not treat or diagnose mental disorders. He/she is qualified to provide psychoeducation and support in the areas of anxiety, ADHD, communication, parenting, mindset adjustments, relationships, behavioral adjustments, life stage challenges/changes, and spirituality.
2. I understand that should my coach detect that I suffer from any mental illness or disorder, he/she will make appropriate referrals.
3. I understand that coaching is not a substitute for counseling, psychotherapy, or substance abuse treatment and I will not use it in place of any form of psychotherapy.
4. I understand that my success depends solely upon me and my willingness to participate in goal setting, homework, and implementation of other suggested tools.
5. I understand that my coach is not licensed by the state of Texas but does have specialized training and certifications in their particular area of expertise. Clients are encouraged to ask questions regarding qualification and limitations of care.
6. I understand that the relationship with my coach may be terminated at any time. Either party may end coaching at any time. Fees for used sessions are non-refundable. Any unused sessions in a package will be refunded only at the coach’s discretion if within the 6-month usage period.
7. I understand that my coach will keep my records and any communication private without permission to release it to specifically designated persons.
8. Coaching is a collaborative, family-focused process. All participating family members agree to engage in the work, take responsibility for their own growth, and contribute to the health of the family system. Progress requires effort from everyone involved.
9. Sessions are scheduled in advance. If you need to reschedule, please provide at least 24 hours’ notice. Missed appointments or cancellations with less than 24 hours' notice may be charged the full session fee
10. Coaching is a private and respectful space. While not therapy, coaching conversations are confidential within legal and ethical limits. With family work, there may be times when information is shared between members with consent to support the process. *If at any time a desire to harm oneself or others is disclosed confidentiality is forfeited. Any disclosed crimes committed against any minor child under 18 years of age will be reported to the appropriate authorities.
Fee Agreement
1. Payment is due at the time of each session.
Accepted payment methods are credit/debit card, cash, or Venmo.
2. Pre-paid sessions must be used within 6 months of purchase.
3. After 6 months, unused prepaid sessions are forfeited and are not refundable or transferrable.
4. Changes in session fees may change from time to time. Clients will be notified in writing (via email) at least 15 days in advance of any fee increases.
5. No-shows and late cancels (appointments cancelled less than 24 hours in advance) will be charged the full session fee or deducted from any prepaid sessions. Emergencies and unforeseen events are normal and can happen from time to time and will be exempt from any cancellation fees. Three unexcused late cancels may result in termination of coaching relationship.
